EXPORTERS DECLARATION OF OWNER PACKED GOODS

I, certify that all shipments being tendered to UPakWeShip / EuroUSA Shipping, Inc. as a freight forwarder / NVOCC described as Household Goods or Personal Effects contain only Household Good/Personal Effect items as defined in *15 CFR 30.38(a),(b),(c.1-5) and that there are no illegal, hazardous, prohibited, or otherwise unclaimed goods contained therein. I further certify that any shipment containing a self-propelled vehicle will be identified and tendered for export in accordance with 19 CFR 192. **(See U.S. Customs and Border Protection regulations 19 CFR 192 for separate CBP requirements for the exportation of used self-propelled vehicles.)

I acknowledge that I have packed all of the items in my shipment.

I confirm that I have full knowledge of all items that I have packed and I have packed them myself.

I confirm that I have not been given anything by another party to ship on their behalf.

I confirm that the information regarding the contents of these packages that I have given to UPakWeShip, Inc. is true and complete.

I confirm that these packages do not contain any illegal or hazardous items, firearms, explosives of any nature or items from endangered species.
